Our Case Management Unit (CMU) is contracted with the Milwaukee County Department of Aging to provide case management services to individuals 60 years of age and older, who live in the community and receive services funded by the State of Wisconsin through the Milwaukee County Department of Aging. We provide on-going care management to between 150-160 individuals (“members”) at any given time.
Our interdisciplinary team, staffed by social work case managers and registered nurses, collaborates with members and those who care about them, to ensure members receive services which will support them in meeting their needs, pursuing their goals and maintaining an optimum level of health, safety and well-being.
We honor and promote each member’s continued contribution to and participation in their community by providing a continuum of quality, cost-effective and long-term options. People using our services might be compromised by health problems, have a disability, or are frail, with limited income. Our CMU team works together with members, enabling them to remain in their homes for as long as possible, with supportive services such as personal and supportive home care, transportation, home delivered meals, medical equipment and supplies.
Should there come a time when, even with support, living independently is no longer a safe option, our team will maintain support and advocacy for an alternative living option which best fits the member and sustains quality of life.
